AHA IMPLEMENT-EF Initiative Adds Pharmacy-Led Cohort to Improve HF Care
Summary: The American Heart Association (AHA) has started a 3-year program called IMPLEMENT-EF to help patients with certain types of heart failure. Now, they are adding a special team led by pharmacists to the program. When heart failure patients leave the hospital, it can be very hard to manage all their new medicines. These pharmacy teams will look at where things go wrong with medicine after patients go home and find ways to fix those problems.
By helping patients understand and take their medicines correctly, pharmacists play a huge role in heart care. Their teamwork helps keep patients out of the emergency room, saves lives, and makes patients feel much better overall.
