Health information management of older, multimorbid patients in German primary care: feasibility and first results of the outcome measures of a cluster-randomised controlled pilot trial - HYPERION-TransCare.
Summary: Imagine going to the hospital, and the doctors there don't have your health records from your regular doctor. This is a big problem, especially for older adults who have many health issues and take lots of pills. Researchers in Germany tested a new way to share this health information better between patients and different doctors.
They did a 6-month test with 12 doctor's offices and 93 older patients. They wanted to see if they could easily track things like hospital visits, falls, and how well patients understood their medicines. The good news is that tracking this information worked very well, and patients were able to answer the surveys without much trouble. The bad news is that the new sharing method didn't actually reduce hospital visits or improve health during this short test. Still, it proves that studying this problem is highly possible for future research!
